Notes: Inspection made, and inquest attended. Whilst leaning over the hoppit repairing the walling of the shaft with another person the winding engine started owing to steam escaping through the throttle valve causing the deceased to be crushed against bearer in the shaft a few feet above. The winder stated he was standing behind the handles with the brake down at the time. On examining the valve, some loose pieces of India rubber were found lying in the valve which might have assisted the escape of the steam.
